Clinical practice forms
- Weekly Conference Form (PDF) – Complete this form each week with the cooperating teacher. One form must be submitted to the supervisor every three weeks minimum.
- Nebraska Clinical Practice Assessment Rubric (PDF) (as of Fall 2018) – This rubric was designed by the Nebraska Department of Education and is used by Nebraska-approved education preparation programs to assess student teachers.
