WSC Student HealthWayne State College Student Health Services is committed to providing the best possible health services for our students. In an effort to do just that, it is mandatory for us to have on file a student health form, a copy of current immunization records, and a meningitis information sheet. To avoid any delays in your registration process please comply with the above request as soon as possible.
1. REQUIRED Immunizations | download Student Health Form Show proof of TWO MMR (Measles, Mumps, and Rubella) Vaccinations OR Show proof of a Positive Blood Titer to Measles (Rubeola)-ATTACH A COPY OF LAB RESULTS. International Students ONLY: Exemption: The Nebraska State College System does have allowances for exemption from immunization. An Immunization Waiver Form can be requested from the WSC Student Health Office pertaining to a medical, religious, or temporary exemption.

Goldenrod Hills Immunization Clinic Goldenrod Hills Community Action is an immunization program that conducts 16 immunization clinics each month in 12 counties in the services area. The clinics are open to the public and there are no income guidelines. No one is denied immunizations for inability to pay. For more immunization information call 402-529-3513.
NESIIS (Nebraska State Immunization Information System) Your know a lot about your child.....favorite foods, favorite color.....But, do you know where your child's immunization records are?
NESIIS is a secure computerized data system that tracks the immunizations given to people in Nebraska. NESIIS is an across-the-lifespan system, which means that everyone, young and old, can keep their immunization information in one secure location. By keeping a record of the immunizations given, health care providers, parents, and legal guardians know when a person is up-to-date on the vaccinations they need.
2. Recommended Immunizations Menactra (Meningitis)Vaccine Hepatitis B Series-A series of three shot usually given in childhood. Hepatitis A Series-A series of two shots or of three if given in combination with Hep B shots. Varicella (Chicken Pox) Series-A series of two shots if no history of Chicken Pox Disease. Tetanus, Diptheria, Attenuated Pertussis-Tdap is preferred OR Td Booster within the last 10 years. Human Papilloma Virus (HPV)-A Series of three shots for females.
