To APPLY for STRIDE

Please contact the STRIDE Office at Wayne State College to request an application form or simply download the application form. Completed applications will be reviewed and our office will contact you with a decision. Complete all parts of the application form along with the income eligibility on page two.

Copies of income tax forms are only necessary for those applicants who would qualify as low income. If you are unsure whether or not your household is low income, please refer to the Application Instructions.

If you have a disability, you must submit written documentation which verifies your disability diagnosis. This information may be obtained from your high school, Vocational Rehabilitation counselor, or other appropriate professional.

Mail the completed application and all other documents to the address below with attention STRIDE/SSS Office.
