for Admission and the Student Health Form) are available to
graduate degree seeking students as Microsoft Word documents.
These forms need to be completed, signed, and forwarded to
the appropriate office at Wayne State College. The Graduate
Application for Admission and the Student Health Form are
.pdf files and can be printed and completed on a typewriter
or in ink (please print legibly).
